Home Observation

You are responsible for making observations every time you visit a client. This may include:

• Physical and mental health – changes in mood, behavior, or body condition
• Home environment and safety – clutter, hazards, or anything unsafe
• Support systems – who helps the client and how often
• Family dynamics – how family members interact and provide care
• Social and cultural factors – traditions, beliefs, or social activities that affect health

Your supervisor will tell you what to look for with each client because every client’s needs are different.
